Is a premium or reusable filter worth paying for?
Only if you will look after it. The calculator's premium or reusable grade costs more than the default, because the part itself costs more. A reusable filter is meant to be cleaned and refitted instead of thrown away, so the extra price is a bet that you will do that cleaning.
Pick the premium grade because you want a reusable filter, not because a shop says it protects the engine better. Ask which grade is on the quote.
How do you tell the filter is the problem, and how urgent is it?
You usually can't from the driver's seat, and the filter is easy to check by eye. Pull it out and hold it to a light. If it is packed with dirt, leaves or bugs, replace it. If light passes through, it can wait.
Sluggish acceleration or a check engine light can point to a clogged filter, but both have many other causes. A new filter is cheap, so it is a reasonable first thing to rule out. It is not a reason to skip a proper diagnosis if the symptom stays. Between the two, the filter is a maintenance item, not an emergency: follow the schedule in your owner's manual.
How do you get a fair quote?
Ask for the filter and the labor as two lines, and ask to see the old filter. The estimate assumes 15 minutes of labor, so a quote that charges much more labor than that deserves a question about why. You can also ask what grade of filter is being fitted, since the premium grade costs more than the standard one.
What does the estimate leave out?
It leaves out anything beyond the filter swap. A diagnosis, a cabin air filter, a cleaning of the air box and any intake part the shop finds damaged are all billed on top. So are taxes and shop fees, which vary by location. If a check engine light is on, expect a separate diagnostic charge, because the filter may not be the cause.

On many vehicles, yes. The filter usually sits in a box held by clips or a few screws, and you pay only for the part. Check your owner's manual for the correct filter, and keep dirt from falling into the intake.
Do I need a new filter at every oil change?
No. Replace it by condition and by the interval in your owner's manual, not on a shop's recommendation alone.
